I have arthritis in both hands and tendonitis in my right arm, and use of a regular mouse is impossible for me. I can use this mouse with either hand, though it does function differently from a regular mouse and takes some getting used to. The first finally wore out after years and years of wonderful service. This is the only type of trackball we would ever use and have recommended it to others. Rating:  Date: 2008-06-28 LOVE this mouse! This mouse is great! It is definitely different from using a regular mouse, but I found it very easy to use, right out of the box. Maybe because I'd read so many reviews here, and been told by someone familiar with the mouse that it was tricky, so I was ready for it. But, I love how it operates - I find it comfortable in my hand, I love not having to pick the mouse up and move it. That makes it easier to work in a tight space, especially when my kids want me to sit in their beds and work while they fall asleep. No more carting a mouse pad and mouse all over the house along with the computer and trying to find space to set it up.
